Wir gucken mal wie schlimm sich nicht-sexuelle
Misshandlung auf Kinder auswirkt.

Man merkt, dass es in diesem Gebiet sehr viele Studien gibt und auch ziemlich gute.
A systematic review was conducted using the Medline, EMBASE, and PsycINFO electronic databases up to 26 June 2012. Published cohort, cross-sectional, and case-control studies that examined non-sexual child maltreatment as a risk factor for loss of health were included. All meta-analyses were based on quality-effects models. Out of 285 articles assessed for eligibility, 124 studies satisfied the pre-determined inclusion criteria for meta-analysis.
Leider berechnet die Studie keine
effect sizes mit dem klassichen .r Korrelationskoeffizienten, sondern nimmt die klassiche
odds ratio, die man eher aus Medikamenten-Studien kennt.
Aber das ist nicht so schlimm. Kurz erklärt: Die
odds ratio vergleicht zwei Gruppen A und B und zeigt an wie wahrscheinlich man von dem gemessenen Effekt betroffen ist, wenn man in A ist im Verhältnis zu der Wahrscheinlichkeit, dass man betroffen ist, wenn man in Gruppe B ist.
Jetzt ein Beispiel aus der Studie:
For example, emotionally abused individuals had a three-fold higher risk of developing a depressive disorder than non-abused individuals (an odds ratio [OR] of 3.06).
Wer also
emotional missbraucht wurde, bei dem war es drei mal wahrscheinlicher später eine Depression zu entwickeln, als in der Kontrollgruppe.
Nehmen wir mal weiterhin Depression als Beispiel. Was erzeugte noch signifikant eine Depression?
Physically abused and neglected individuals also had a higher risk of developing a depressive disorder than non-abused individuals (ORs of 1.54 and 2.11, respectively).
An zweiter Stelle ist Vernachlässigung und erst an dritter Stelle physische Gewalt in Bezug auf Depressionen, jeweils doppelt und eineinhalb so wahrscheinlich mit diesem Hintergrund eine Depression zu entwickeln.

The relationship between abuse and neglect in childhood and subsequent health effects is complex. Although childhood abuse and neglect does result in adverse health outcomes, these outcomes are not independent of broader socioeconomic contexts. Lifestyle factors, access to health care, and neighbourhood characteristics may act as mediators between child abuse and neglect and long-term health consequences. Exposure to child maltreatment often co-occurs within the context of other family dysfunction, social deprivation, and other environmental stressors that are also associated with mental disorders. Child maltreatment may be a marker of other family problems that together lead to the development of mental disorders. In addition, findings from many studies do not take into account the likely contribution of hereditary influences on the predisposition to mental disorders. Children of depressed parents may be at greater risk of depression through both exposure to maltreatment by their parents and genetic predisposition Hence, some of the effect of child abuse and neglect on mental disorders may still be explained by confounding. However, the effect of abuse on mental disorders remained significant in the majority of studies included in these meta-analyses after controlling for these co-occurring factors.
Da sei noch hinzugefügt: Man kann auch nur die konfundierenden Variablen kontrollieren, die man kennt.
Bezüglich keiner Kausalität-Analyse geben die Autoren folgendes zu Bedenken:
In summary, there was robust evidence of significant associations between exposure to non-sexual child maltreatment and increased likelihood of a range of mental disorders, suicide attempts, drug use, STIs, and risky sexual behaviour. An increase in the likelihood of alcohol problem use was not consistently seen. There is weak to limited evidence suggesting a relationship between non-sexual child maltreatment and certain physical disorders and risk factors (Table

but more research is required to confirm these relationships.

Ungeachtet dessen, dass natürlich - wie fast immer - eine Prise Skepsis und Gewissheitseinbußen zu bedenken sind, sieht die Datenlage ziemlich solide aus: Kinder, die verschiedene Formen von non-sexual
maltreatment durchlebt haben, haben später mit höherer Wahrscheinlichkeit
adverse outcomes im Erwachsenenalter.

Wieder eine Metaanalyse, diesmal von 43 Studien, die folgendes untersuchen:
In the context of PARTheory, this perspective asserts that humans
have developed over the course of behavior-genetic coevolution the enduring biologically based
Parental Acceptance and Adjustment emotional need for positive response from the
people most important to them. According to the theory, this need in childhood is for parental affection,
care, comfort, support, nurturance, or simply love (acceptance). The need becomes more
differentiated and complex for adults, to include the wish or yearning (recognized or unrecognized)
for positive regard from people with whom one has an affectional bond of attachment (Rohner, 2001a).
PARTheory also postulates that when this need for positive response is unmet by significant
others, humans have the phylogenetically acquired tendency to develop a specific set of socioemotional
and cognitive dispositions specified in personality subtheory (Rohner, 1999a).

Die Autoren kommen zu dem Ergebnis, dass:
These results lend strong support to PARTheory’s assertion that increases in perceived acceptance-
rejection are universally associated (among both children and adults) with increases in psychological
maladjustment.
Aber natürlich auch mit den jeweiligen Reservationen, dass es sich um keine
longitudinal studies handelt, also die befragte Kohorte nicht verfolgt wird um psychological adjustment in einem
Follow-Up erneut zu befragen.
Auch handelt es sich hier um
perceived rejection. Dagegen gab es aber auch einige Studien, die auch externe Befragungen (Lehrer, Sozialarbeiter) und andere psychologische Measurements durchgeführt haben, welches mit PARTTheory korreliert.
Wir haben also in vielerlei Hinsicht eine ähnliche Situation zu sexuellen Missbrauchsstudien: Die Befragung der Inzidenz von sexuellem Missbrauch im Kindesalter und jetziges adjustment.
Es gibt allerdings noch einen Unterschied in der
Phänomenlologie: Wenn Kinder befragt wurden, dann gab es keinen
Interventionseffekt - also das adjustment von befragten Kindern war vorher genauso gut bzw. schlecht wie nachher. Klar: Es gibt auch keine wirklichen
Interventionsmoment, anders als bei kriminalisierten und oftmals jahrelang geheimgehaltenem sexuellen Missbrauch.
Die Effektstärken, die man bei sexuellem Missbrauch findet, verrate ich noch nicht.
Bevor ich im dritten Beitrag zu den effect-sizes von CSA kommen, noch eine kleine Überleitung.
Ney et al. (1994) haben sich die schlimmsten Kombinationen verschiedener Formen von
child abuse angeguckt. Anstatt in einer Studie nur einzeln bestimmte Formen von abuse einzeln zu messen und herauszurechnen, war die Fragestellung: In welchen Kombinationen tritt eigentlich child abuse so auf? Und welche Kombination (kann ja auch etwas einzelnes sein) war für Kinder im Endeffekt am schädlichsten?

Zunächst einmal fand man heraus, dass weniger als 5% aller Formen von
child abuse allein auftraten. Es waren also quasi fast immer Kombinationen verschiedener arten von Missbrauch/Misshandlung.
Wie immer muss man natürlich vorsichtig sein für wie Gewiss man die Ergebnisse halten kann. Oftmals geben solche Studien trotzdem ganz gute eine Richtung an, in der man weiterforschen kann.

These data come from a long-term research project that followed 167 children and adoles- cents from 7 to 18 years of age. Eleven children were selected from a private psychiatry practice, 107 from the Arbutus Society Adolescent Unit in Victoria, British Columbia, 23 from the Calgary Young Offenders Centre, and 26 from a Victoria high school. Each child was interviewed, or, if older, asked to complete the Child Experience Questionnaire himself. The answers involved checking the appropriate selection on a list, or drawing a mark across a 9cm Visual Analog Scale (VAS), for example: My enjoyment of living is: very low/very high
Stichprobe also nicht sehr groß repräsentativ (ein Großteil klinisch), es handelt sich auch um self-reports von Kindern/Jugendlichen (Zur zeit als Kind/Jugendlicher)
Die Stärke: Direkter Vergleich aller vorkommenden
maltreatements bei den selben Kinder/Jugendlichen.
Die schlimmsten Kombinationen sind:
Ranked according to the average correlation, and beginning with the most devastating combination, verbal abuse appears seven times in the top 10, physical neglect six times, physical abuse five times, emotional neglect five times, sexual abuse once.
Sexueller Missbrauch kommt in den Top-10 der schlimmsten
maltreatments nur ein mal vor.
Bemerkenswert ist auch folgende Beobachtung:
Among the first 10 combinations of abuse and neglect, ranked as correlated with the child’s view of his/her future, 9 had a combination of abuse and neglect. This plus other data yet to be published seem to indicate that a combination of neglect and various forms of abuse makes the child particularly vulnerable. We have found that there is a significant association between the extent of abuse and whether the neglect occurred before or at approximately the same time or later than the abuse. It appears that children who are first neglected are more vulnerable to being abused. Metaphorically speaking, children who are starving will eat anything, even if it’s garbage off the street. They are hungry for affection, but only look for it in those who have little to offer but more abuse. This might explain why, although sexual abuse does not appear to have a prominent effect on the measures we have used, when in combination with emotional neglect it is significantly harmful.
Heißt:
An sich scheint sexueller Missbrauch kaum einen Effekt zu haben -
aber in Kombination mit emotionaler Vernachlässigung hat es einen Effekt.
Macht ja auch Sinn: Zusammen mit einem Erwachsenen, der das Kind
wie Müll behandelt, kann es nur gefühllosen und rücksichtlosen Sex ergo sexuellen Missbrauch geben, der eben nicht positiv empfunden werden kann. *duh*
Ach ja, noch eine kleine Randnotiz: Bei
sexual abuse wurde auch immer "
against your will / forced / when you did not want to" gefragt.
Das heißt: Auch in diese Richtung könnte noch ein bias drin sein.
Jetzt kann man natürlich - zu Recht - sagen, dass der Schaden ja erst auch als Erwachsener kommen könnte und Sex (sogar auch gegen den Willen) relativ unschädlich als Kind ist.
Dazu kommen wir dann ab dem nächsten Beitrag.
